Type 2 (non-insulin dependent) DM.

Dosage/Direction for Use
Initially 500 mg, may be adjusted after 2 wk. Max: 2,000 mg daily. Consider other suitable oral prep for recommended dosages other than 750 mg. Patient w/ renal impairment (GFR 30-44 mL/min) Max: 1,000 mg daily.

Overdosage
View Metphar XR 750 overdosage for action to be taken in the event of an overdose.
Administration
Should be taken with food: Take w/ the evening meal. Swallow whole, do not chew/crush.
Contraindications
Hypersensitivity. Liver problems; severely reduced kidney function; uncontrolled diabetes w/ severe hyperglycaemia, nausea, vomiting, diarrhea, rapid wt loss, lactic acidosis or ketoacidosis; dehydration; severe infection eg, infection of the lungs or bronchial system or kidney; previously treated for acute heart problems, recent MI or presence of severe circulatory problems or breathing difficulties; heavy drinker of alcohol. Childn <18 yr.

Special Precautions
Risk of lactic acidosis. Temporarily interrupt treatment in case of conditions associated w/ dehydration eg, severe vomiting, diarrhea, fever, exposure to heat or drinking less fluid than normal. Intravascular administration of iodinated contrast agents; discontinue treatment prior to or at the time of imaging procedure & do not restart until at least 48 hr after. Discontinue treatment during & for some time after a major surgery. Check kidney function at least once a yr or more frequently if clinically indicated. Caution when driving or operating machinery if taken w/ other antidiabetics. Patients w/ renal impairment. Do not take if pregnant or breastfeeding. Elderly.

Adverse Reactions
Diarrhoea, nausea, vomiting, stomach ache or loss of appetite. Taste disturbance. Lactic acidosis; abnormal liver function tests, hepatitis.
Drug Interactions
Contrast-induced nephropathy resulting in metformin accumulation w/ iodinated contrast agents. May require frequent blood glucose & kidney function tests or dose adjustment when co-administered w/ diuretics (eg, furosemide); NSAIDs & COX-2 inhibitors (eg, ibuprofen, celecoxib); ACE inhibitors & angiotensin II receptor antagonists; steroids eg, prednisolone, mometasone, beclomethasone; sympathomimetic medicines eg, epinephrine & dopamine; medicines that may affect metformin blood levels (eg, verapamil, rifampicin, cimetidine, dolutegravir, ranolazine, trimethoprim, vandetanib, isavuconazole, crizotinib, olaparib). Increased risk of lactic acidosis w/ excessive alcohol intake.
